If the information you submitted matched a MyThedaCare account in our records, then your username has just been sent to the email address on file. If you do not receive an email with your username, then please contact your provider's office for assistance.

If you have an e-mail address on file then your MyThedaCare Username will be sent to your e-mail account.
If you do not remember any of this information, or you do not have a valid e-mail address on file, you will have to contact your provider's clinic to help you regain access to your MyThedaCare account.
